    Our Organisation

    South Eastern Melbourne Primary Health Network (SEMPHN) commissions services and works in partnership with local stakeholders to help people in south east Melbourne get the health care they need, when and where they need it. We are leaders, facilitators and influencers in the shared goal of better primary health care.

    Governance

    SEMPHN is governed by a Board of Directors who are supported and advised by a Governance Committee and a Finance and Audit Committee. Our Clinical and Community Councils regularly provide the Board with further insight into areas of need in primary health within the catchment, from both a clinical and community-based perspective.

    Understanding the Health Needs of our Community

    As a PHN, we work to invest in services that are evidence-based and meet the needs for our most vulnerable communities.

    Digital Innovation and Quality Improvement

    SEMPHN is committed to providing digital health solutions to help general practices and the broader community

    Working Towards a Healthier Community

    SEMPHN is continuing to work with general practices and other partners to improve chronic disease management through a range of commissioned programs aimed at preventing and managing chronic health conditions across the region.

    Integrating Health Services to Support Aboriginal and Torres Strait Islanders

    SEMPHN continues to partner with general practices, providers and local communities to improve health outcomes for Aboriginal and Torres Strait Islander people with chronic health conditions through better access to care coordination, multidisciplinary care, and support for self-management.

    Reducing Harmful Impacts From Alcohol and Other Drugs

    In the last year, SEMPHN invested $4.2M to commission a wide range of Alcohol and Other Drug (AOD) services and activities to support vulnerable Victorians across the region.

    Improving Patient Access to Pharmacotherapy Services

    The Area 4 Pharmacotherapy Network (A4PN) works to improve patient access to medication to assist in the treatment of opioid addiction (known as pharmacotherapy) across southern and eastern metropolitan Melbourne.

    Services to Support Young People in Our Community

    SEMPHN commissions primary mental health services within a person-centred stepped care model. We aim to provide youth-friendly services that are responsive to the varying mental health needs of young people and their families in our region.

    Preventing Suicide in our Community

    SEMPHN is passionate about leading and contributing to discussions around the importance of safe language in suicide prevention, working with those with lived-experience of suicide to inform suicide prevention and implementing evidence-informed approaches.

    Increased Access to Better Mental Health Care

    SEMPHN commissions services to help address the mental health needs of our community, including people with mild, moderate and severe mental health needs.
    These services range from peer-support, to brief psychological sessions, to individual packages of care. These services form part of our Mental Health Stepped Care Model, which tailors care to individual need and illness severity.

    Connecting Our Community to Appropriate Services

    SEMPHN’s Access & Referral (A&R) team continue to provide valuable support to GPs and service providers by providing a clinical, first point of contact for low-income communities who require access to SEMPHN’s commissioned services, as well as other Mental Health, Alcohol and Other Drugs (AOD) and community supports.
